NOTE: This form is used to notify Macy's, Inc. of a vendor's migration to VICS 4010. All divisions and transaction sets will be migrated simultaneously. The MST EDI Support Center will notify the vendor for verification of information before execution and upon completion of the request. All migrations will be completed within five days of verified information. Parallel testing is not available. All fields required unless specified.
